To book our services, follow the three simple steps below:

1) Use the information to fill out your invoice form (click here to download the proforma invoice template). Instructions on how to fill the invoice is listed below. If you are okay with the cost, then proceed to:
2) Fill out our "Engagement Form" and submit. Follow this LINK to fill out the form. A copy will be sent to your email after submission.Â
3) Email us your completed Engagement Form and Profoma invoice.
a) Send To: admin@carehopes.com
b) Email Subject: Companionship Service Invoice, and we will review your application and get back to you within 5 working days.Â
To use the proforma invoice template, follow the simple steps below:

1) Only type in the white columns at the right side of the template. You will not be able to type elsewhere.Â
2) Select from drawdown where applicable
3) All the information you need are available in the left side of the template
4) Your total cost will be displayed by the template.Â
5) Save the template and send back to us. You may wish to rename it.Â
