Who we work with
Supporting professionals and families
CAREHOPES welcomes referrals from a wide range of professionals and organisations. Whether you are a social worker, occupational therapist, GP, discharge coordinator, or community nurse — if you have a client or patient who would benefit from regular companionship and social support, we would like to hear from you.
We also work directly with family members and unpaid carers who are looking for trusted, reliable support for a loved one. You do not need to go through a professional to refer someone to us.
Health & Social Care
Social workers, community nurses, occupational therapists, discharge teams, and care coordinators looking for companionship support for clients.
Family Carers
Family members and unpaid carers seeking trusted, regular support for a loved one — including respite cover to give carers a break.
Care Providers
Residential homes, supported living providers, and day centres looking to supplement their offer with community-based companionship.

Charities, voluntary organisations, and community groups who want to connect their members with additional wellbeing support.

Complete the referral form below or contact us directly. Tell us a little about the person and what kind of support they need.
Initial conversation
We will contact you within one working day to discuss the referral and answer any questions you have.
Assessment & matching
We carry out a person-centred assessment to understand the individual's needs, preferences, and routines — and match them with the right companion.
Support begins
Once matched, we introduce the companion and begin support. We keep you updated and welcome ongoing communication.
Working together
Partnership opportunities
We are always open to exploring formal partnerships with organisations that share our commitment to improving wellbeing and reducing isolation. If you represent a health trust, local authority, housing association, or voluntary sector organisation, we would welcome a conversation about how we might work together.
